When calling to schedule your first appointment you will be asked for the following information:

• Name, address, phone, and social security number
• Insurance information (please have your card available)
• Information about your injured knee including: how, where and when the injury occurred

First Appointment

Please arrive 15-30 mins before your scheduled appointment. If you have not pre-registered, you will be asked to fill out your registration forms. If you faxed or emailed your forms ahead of time, we will use this time to double check for accuracy and gather any missing information.
