Check-in and Check-out

The front office is located on the 6th floor, to your left as you exit the elevator. This is where each appointment begins and ends.

As you check in, you will be asked to provide and/or verify your information. Please make any corrections regarding your personal and insurance information at this time.

As you check out, your next appointment, if needed, will be made, co-payments and payments will be collected at this time.

Co-payments

Co-payments will be collected when you check out. You may be required to pay a second co-payment if you are treated in our physical therapy department. Physical therapist, like doctors are considered medical providers by insurance companies.

Gym Shoes
Shorts
Completed Registration Form
Current Insurance Card
Any medical records pertaining to previous knee injuries and/or surgeries
X-rays, MRI's
Photo ID

As you check in for your follow up appointment (if needed), please notify the front office of any changes in your information (insurance, address etc) at this time.
